Five years ago NACCHO published the Continuity of Care in Breastfeeding Support Blueprint. More than 45 counties and communities have since put it to work.

Breastfeeding Family Friendly Communities presents alongside NACCHO, the Black Mothers’ Breastfeeding Association in Michigan, and the Augusta University Research Institute in Georgia. Three very different places, applying the same framework, learning different things.

What we will actually talk about is the unglamorous part: how you get a health department, a community organization, and a hospital to hand a family off to one another without dropping them.
